Mary Vasquez lives in Dallas, Texas. A cancer and heart attack survivor, suffering from diabetes and a raft of other ailments, she works at Wal-Mart because she cannot afford to retire. Of the $23,000 she earned in 2011, nearly $2,500 went to health insurance premiums. Yet when she needed a pacemaker, she had to pay $4,000 out of pocket, and when an ambulance has to take her to hospital she is billed $600. She takes out payday loans to pay her rent and routinely misses meals.
